We will connect with all parties to ensure care needs are being met post-discharge.<\/p>\n<p>Our caregivers act as an extra layer of support between your loved one, their family, and their healthcare team. We focus on the small but important details that often make the biggest difference \u2014 things like:<\/p>\n<ul>\n<li>Medication reminders, and help stay on track with instructions.<\/li>\n<li>Meal preparation and hydration support \u2013 a key to recovery.<\/li>\n<li>Fall prevention through standby assistance and a tidy, safe home environment.<\/li>\n<li>Transportation and appointment reminders, so follow-up visits aren\u2019t missed.<\/li>\n<li>Encourage Physical Therapy and Occupational Therapy exercises are performed on days when in-home PT or OT is not there.<\/li>\n<li>24\/7 or around-the-clock care available! Someone can be awake and immediately available to be with your loved one during all hours of the day AND night.<\/li>\n<li>Provide real-time updates to family and care providers.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Our caregivers and Client Care team help stay closely connected, and we often notice early signs of a problem \u2014 before it becomes an emergency.
