- Personalized Support We provide needs assessments to develop tailored care plans.
- Crisis Assistance We help during urgent situations through our Emergency Contact Program.
- Coordinated Care We can be the central point of contact for medical appointments and communication among family members and the healthcare team.
- Smooth Transitions We help navigate the transition home from an ER visit, hospitalization, or rehab stay.
- Family Support We provide training and consulting to help you become a more confident care partner for your loved one living with Alzheimer’s Disease or another Neurocognitive condition.
Our Care Management professionals can also provide referrals for senior living placement services, medication reviews, nursing/PT/OT/hospice/palliative care services, and medical, legal, and financial professionals as a complimentary service.
- $110 Per Hour for the services of our Professional Care Managers & $50 Per Hour for the services of our Care Manager Assistants
