How Home Care Helps Prevent Falls

1. Ongoing Home Safety Monitoring:
Most homes were never designed for a 90-year-old with balance issues. Throw rugs, poor lighting, clutter, narrow hallways, low beds / couches, lack of equipment or improper usage of equipment, and slippery bathrooms are all issues that make a big difference in keeping someone safe at home.
Sometimes prevention is simple. Remove the rug. Add the grab bar. Buy the equipment. Other times, it requires a person to be there regularly prompting and watching for hazards.
One of the most important things our caregivers do at Touching Hearts at Home is provide ongoing home safety oversight. That means identifying risks before they turn into injuries. Our staff can identify concerns for families to address, they can point hazards out to clients as they walk, they can remind or prompt seniors to use the equipment provided.
2. Standby Assistance with Hygiene & Transfers:
The bathroom is an area where the risk of falls may be the greatest. Transfers in and out of the shower, getting on and off the toilet, and wet floors all present risks for falls.
Standby assistance with bathing, toileting, dressing, and ambulation is not about taking away independence — it’s about protecting it. Some of our clients just need some stabilization and to be handed things to make the process easier. That steady arm at the right moment makes all the difference.
We can also provided standby assistance while clients are walking – whether it’s staying an arm’s length away OR as a contact guard (always having a hand on). Standby assistance can come in the form of dressing assistance or transfer assistance (from bed to wheelchair, bed to walker, wheelchair to walker, etc.). That steady presence often prevents the one misstep that changes everything.
3. Reducing Overexertion & Unsafe Tasks:
Many falls happen when someone tries to do “just one more thing.” Carrying laundry down the stairs. Making that last grocery bag trip to the car, reaching for that high shelf, bending low to clean the bathroom or dust that low area.
Our, caregivers take on some of those more physically demanding tasks so seniors don’t have to. Many clients help us with these processes so do it just the way you like! Fall prevention isn’t about limiting independence — it’s about eliminating unnecessary risk and saving energy for the things you love to do!
4. Medication & Monitoring Matter:
Missed medications or medication non-compliance are major contributors to senior falls. These missed doses or doses taken at the wrong day or time can lead to dizziness, confusion, or low blood pressure.
Our caregivers provide medication reminders, record keeping, and monitoring for side effects. Medication compliance and observation are key components of reducing hospital readmissions — and falls are often at the center of that conversation. When someone is home alone all day, small changes go unnoticed. When a caregiver is present, those changes are caught much quicker. Often times that leads to an urgent care or primary care visit instead of an Emergency Room or Hospital stay!
5. Ongoing Case Management & Oversight:
Fall prevention is not static. A senior’s strength, balance, and cognition can change gradually or suddenly. Our Client Care Manager provides free regular home visits (both formal and informal). We routinely review care plans, monitor caregiver notes for changes in condition, provide supervision for our care team and guidance for our clients and their families based on our professional recommendations.
