Every day, case managers help seniors and their families navigate some of life’s most complex moments — hospital stays, rehab transitions, and the move back home. They coordinate care, advocate for patients, and bring calm and clarity when things feel overwhelming.
At Touching Hearts at Home Denver Metro, we’re spending the week delivering meals to many of our hospital and rehab partners to say thank you — not just for what you do, but for how you do it. With compassion. With patience. With heart.
